Information on making such requests should be accessible on the school’s webpage with paper copies available from the school office. The Absence Request Due to Exceptional Circumstances form must be filled in by the parent and submitted to the Head Teacher.
Requests must be made in advance of the period of absence. Each case should be reviewed in consultation with the Head Teacher and school Attendance Lead. The school can authorise requests for a maximum of 10 school days beyond this authorisation must be provided by Education Wellbeing Service Team Leader.
